Feedback forms can help you keep your interviews more structured. They are created under your company settings and it is up to you what questions they should contain, and how the forms should be used.

One way to use feedback forms is to include a number of prepared questions that you know that you want to ask the candidate, which will help you keep your interview more structured. Don't forget to make sure to ask the same type of questions to all candidates in the process, as the point of a structured interview is that you should be able to compare the candidates' answers with each other.

Another way of using feedback forms is to list the different requirements you are looking for in a potential hire, in that way you can for each requirement track if the candidate fulfills the requirements or not.
